When Concorde flew at supersonic speeds, the aircraft skin expanded by several inches. In the cockpit, this produced a gap between the Flight Engineers panel and the bulkhead. On the last ever Concorde flight, this cap was placed in the gap, so that it became trapped forever more on the flight deck of 216.
